To complete this claim form:

• Fill in all of the fields in the claim form
• Right-click within the document and select “Save as…”
  OR
  Click CTRL+S to bring up the Save dialog box
• Save the document on your PC
• Email the completed claim form to traveldept@mapfre.com

We will send an acknowledgment letter to you upon receipt of your completed claims form which will advise you of your unique claim reference number. Please make sure you quote this reference number in any future contact you have with us.
